National Identity Card

Prepare the first identity card

1. Application of the Department of Registration of Persons
2. Birth Certificate And Certified copy .(a free issued copy is not valid)
3. Photographs according to the ICAO standard
4. A stamp fee Rs. 140/-
5. If elder than the age of 16 a fine receipt of 2500/-
6. .Cash receipt to the effect that payments have been made to the Grama Niladhari (Rs.200/-)

 (These applications should be obtained from the Grama Niladhari and handed over to the Divisional Secretariat)

Amending the Identity Card

1. Application of the Department of Registration of Persons
2. Birth Certificate And Certified copy .(a free issued copy is not valid)
3. Photographs according to the ICAO standard
4. The Identity presently used
5. Stamps Fee Rs. 140/-
6. Cash receipt to the effect that payments have been made to the Grama Niladhari (Rs. 500.00)

(These applications should be obtained from the Grama Niladhari and handed over to the Divisional Secretariat)

Preparation of lost identity card

1. Application issued by the Department of Registration of Persons.
2. Birth Certificate And Certified copy .(a free issued copy is not valid)
3. Photographs according to the ICAO standard
4. stamp Fee Rs. 120/-
5. Copy of complaint made to the police station(complaint not over 6 months)If an Identity card is lost the owner of the ID Should report to the nearest police station and the application should be submitted with the certified copy of the relevant extract.
6. Cash receipt paid to the Grama Niladhari (Rs. 1000.00)
7. Two Documents for verifying that the number indicated mentioned in the lost identity has been used.

(These applications should be obtained from the Grama Niladhari and handed over to the Divisional Secretariat)

Getting Birth/Marriage/Death Certificates

Obtaining copies of birth, death and marriage certificates

1.Application

2.Fee for a copy

      In case the registration number or date is given - Fee per 01 copy – Rs.120.00

      In case the registration number or the date is not known – Fee for a copy – Rs. 250.00

Belated registration of birth/death  - Section 24/36

1.Application

2.Results of searching for documents to ascertain that the birth/ death has not been registered.

3.Affidavit

4.Recommendation letter from the Grama Niladhari

5.Documents to confirm the birth/ death has been occurred

   (Hospital birth/ death report/ Births/ deaths reported by Grama Niladhari (original copy)

6.Marriage certificate of the parents/ Birth certificate of the father/ mother (for births)

7.Documents to confirm the identity of the declarent

8.Fee for Declaration  Rs.60/-

Amendment of Names – Section 27

1.Application

2.The relevant birth certificate

3.News Paper Advertisement (if below than the age of 21)

4.Recommendation letter from the Grama Niladhari

5.Evidences to the effect that the name has been in use for the period more than one year (minimum of 3 years) (if the persons age is below than 7 years the marriage certificate of the parents)

7.Certified copy of the National Identity Card of the Declarent

8.Fee for declaration – Rs. 60/-

Father’s information, marital status, alteration of mother’s maiden name after getting married, mother’s information and correcting other incorrect information. (section  - 27 (a) / 52 (1)
